I really wish there was a visual novel game that features an ugly female protagonist. I mean, I still enjoy playing as cute girls too because I get to live vicariously through them, but it would just be nice to play a cute romance game with an ugly girl. It would be easier for me to self insert kek. I'm honestly debating making one on my own but idk where to start. And it wouldn't be just romance centered either. Just an ugly girl with a cute life and wouldn't have to worry about judgments and insults and shit like irl.

Otometeki Koi Kakumei Love Revo (an otome game) features a female protagonist who is fat and ugly, but the goal of the game is to lose weight :/ That's the closest I can think of.

On the western side, there's Sword Daughter, a really short otome game on Steam that has a very western art style (not in a bad way, though). Tyrna the protagonist isn't exactly what I'd call "ugly", but rather, a bit "plain", although some would say the art style makes her look ugly simply because they think realistic = ugly. The main love interest is a pretty elf guy, but to be honest, the romance elements aren't very strong, but I liked it a lot nonetheless. Iirc it's an adaptation of a real choose your own adventure book or something like that, which imo, gave it a distinct feel different from your average anime-inspired OELVN.

There's another otome OELVN called Yrsa Major where the protagonist is a woman who's not conventionally attractive. She's a big and strong lumberjack who wants nothing but to protect her poor village. Although you probably won't like it if you hate unconventionally attractive submissive ojisan characters kek. It's an absurdly specific niche that caters to my preferences, but I know there's plenty of women who would find it disgusting (although to be fair he's a magical being that took on that form for no particular reason other than being the dev's fetish, so he isn't really an older man). If you don't mind this though, you'll probably like it because it's an extremely comfy VN!

Don't know if you're interested but there have been a lot of indie Japanese otome games (many of them free) where you can't see the protagonist at all (or you can only see their arms sometimes), POV-style. But of course you're gonna have to know Japanese or use something like VNR to read them lol.

Start by writing the story, worry about the art, music and programming later. Making a simple VN on Ren'Py is easy as shit.

Would the story be about this (being ugly and living a comfy life despite that)? If not, you'd need to think of an actual plot, or some sort of conflict. Of course, the conflict doesn't necessarily have to have anything to do with the protagonist's appearance, she can just happen to be ugly/plain.
